You agree that within one month of the camp completion the applicant (Athlete) must submit a report outlining what they did at the camp, how it benefited them, and what they liked most about the camp.

Please include a completed Camp Registration Form. If this is not available, you can provide a confirmation letter/email from the camp.

Coach Endorsement/Support Letter. Ideally it should be from an NWT Coach, but it can be from the camp coach. The coach’s letter, at minimum, should include the following:

• Coach Name, Phone number, Email

• How long have you coached this athlete?

• How does this athlete exhibit fair play and sportsmanship?

• Why would this athlete benefit from the program?

• How does this athlete exhibit high performance?

• Please describe the character of the athlete

• Is this athlete part of a High-Performace

• Any other information
